Security Freeze

Placing a security freeze on your file is a way to control access to your information since it prevents SageStream from releasing your credit report or score without your consent.

In order to place a security freeze on your file, you will need to submit a written request to SageStream that includes the information listed below.  For security purposes, we must verify your identity by receiving a minimum of two copies of verification documents that match the information you provide with the request.  SageStream does not charge a fee to place a security freeze or to remove one from your file.

Please provide the following information to request a security freeze on your file:

1. A written request with signature that includes the following information:

  1. First name, middle name, last name, suffix (if applicable)
  2. Current address including ZIP code
  3. Current phone number with area code
  4. Social Security number
  5. Date of birth (month, date and year)

 

2.     Copies of at least two of the verification documents listed below that substantiate the personal information that you provide as listed in Step 1:

  1. Current state-issued driver’s license or other current government identification (such as a passport)
  2. Recent cable, utility or phone statement, produced 60 days ago or less
  3. Social Security card
  4. Birth certificate
  5. U.S. passport picture page
  6. Alien registration card
  7. Voided personal check with your preprinted address

Your request will be processed within three business days after receipt.  A confirmation letter will be sent by USPS first-class mail and will include a personal identification number (PIN) issued by SageStream. This PIN will be required to remove the security freeze.

 Remove the Security Freeze

It is important to remember to remove the security freeze before you plan on applying for credit or opening new accounts such as wireless, cable or satellite utilities. Otherwise, you may experience unexpected delays in establishing new accounts until you can contact IDA, Inc. and provide the necessary information to remove the security freeze.

To remove the security freeze, you will need to provide both of the following:

  1. Verification of the personal identifying information that you submitted at the time of your request, and
  2. The PIN issued to you in the confirmation letter sent by SageStream

You can request removal of the security freeze by mailing your request to the above address or by calling SageStream toll-free at (888) 395-0277 Monday – Friday, 9:00 AM to 5:00 PM (Pacific Time).

Block Information as a Result of Identity Theft

If you believe specific information in your consumer report is incorrect as a result of identity theft, you can request that the fraudulent information be blocked from your report. Blocking the information will not prevent SageStream from releasing your credit report or score to organizations that have a legal right to receive it, but it will prevent the specific information that you believe is related to identity theft from being included in the report.

In most cases, it is recommended that you first obtain a copy of your consumer report from IDA, Inc. to review the information we currently have about you.  Instructions on how to obtain a copy of your consumer report can be found on the Consumer Report page.

To request that specific information related to identity theft be blocked from your consumer report, you will need to return a copy of your report indicating the fraudulent information that you want to have blocked, and you will need to provide us with a copy of your Identity Theft Report.  For information about Identity Theft Reports and how to create one with the Federal Trade Commission (FTC), refer to the FTC website at: http://www.consumer.ftc.gov/articles/0277-create-identity-theft-report.
