If you submit a written request by secured fax or mail, please provide the following information:

B. Proof of Identity Documentation
For security purposes, to confirm your identity, please include with your written request a legible copy of your current driver’s license or other current government-issued identification card showing your current address and date of birth.
If your driver’s license or other government-issued identification card is not current or if it does not show your current mailing address, then a legible copy of a document from Category 2 listed below is also required. Further, if you do not have a driver’s license or other government-issued identification card, then a legible copy of a document from Category 1 and Category 2 listed below are required.
Please note that the address on the identification card or other identity verification document must match the address provided in your request.
When we receive your dispute, we will reinvestigate the disputed information free of charge. If the results of our reinvestigation show that the information is inaccurate or if it cannot be verified, we will modify or delete the information from your SageStream consumer file. If our reinvestigation confirms the information is accurate, the information will remain in your SageStream consumer file. You will receive a notice with the results of the investigation along with your updated SageStream consumer report usually within thirty (30) days. After you file a dispute, if you provide additional information that is relevant to your dispute, the time required for completion of our reinvestigation may be extended up to fifteen (15) days.
Although not required, we recommend that you obtain your SageStream consumer report prior to filing a dispute so that you can review all of the information contained in your report for accuracy and completeness.
