Consumer Report

SageStream, LLC (“SageStream”) is a consumer reporting agency that prepares consumer reports and credit scores for its clients who are typically lenders and other credit-granting companies.

It is always a good practice to obtain a copy of your consumer report, also referred to as a “credit report,” a “consumer file,” or a “disclosure report”, to know what information is being reported about you and to ensure that the information is accurate.  Requesting your SageStream consumer report will not affect your ability to apply for or to obtain credit.

If you would like to obtain your SageStream consumer report, you may:

  1. Submit your request securely, online by completing our Consumer Report Request Form;
  2. Submit a written request by secured fax at (858) 451-2847 or by mail to: SageStream, LLC Consumer Office, P.O. Box 503793, San Diego, CA 92150;

For security purposes, we must confirm your identity.
If you submit a request online or by secured fax or mail, please provide the following information:

A. Required Information

      • Your first name, middle name, last name, suffix (if applicable)
      • Your current address including ZIP code
      • Your current phone number with area code
      • Your Social Security number
      • Your date of birth (month, day, and year)

B. Proof of Identity Documentation

For security purposes, to confirm your identity, please include with your written request:

    • a legible copy of your current driver’s license or other current government-issued identification card showing your current address and date of birth.
      NOTE: You will be required to submit additional identity verification documentation under the following circumstances:

      1. If your driver’s license or other government-issued ID is not current or does not show your current address and date of birth:
        • Submit a legible copy of a document from Category 2 listed below along with a legible copy of your driver’s license or other government-issued ID.
      2. If you do not have a driver’s license or other government-issued ID
        • Submit a legible copy of a document from Category 1 and Category 2 listed below.

  • U.S. passport picture page
  • Alien registration card
  • Military identification card (front and back)
  • Recent cable, utility or telephone statement with your name and current mailing address
  • Bank or credit card statement with current address
  • Rental lease agreement showing current address
  • Voided personal check with your preprinted current address
  • Notarized letter from your landlord confirming your current name and residential address
  • Mortgage statement with the current address
  • W-2 tax form with your current address

Please note that the address on the identification card or other identity verification document must match the address provided in your request.

Your request will be processed within seven (7) business days (or as required by applicable state law) after we receive all the required information, and your consumer report will be sent by USPS first-class mail.  SageStream does not charge you for providing you with a copy of your SageStream consumer report.
