Blocking Fraudulent Information

If you are a victim of identity theft, you can block the reporting of information contained in your SageStream, LLC (“SageStream”) consumer report that resulted from the identity theft (the “Fraudulent Information”).

To block the Fraudulent Information from being reported on your SageStream consumer report, please submit the following information to us by secured fax at (858) 451-2847, or by mail to: SageStream, LLC, Consumer Office, P.O. Box 503793, San Diego, CA 92150:

1. A copy of your identity theft report that you filed with a law enforcement agency. Law enforcement agencies include, but are not limited to:

  • Police Departments
  • The Federal Bureau of Investigation
  • States’ Attorneys General
  • U.S. Secret Service
  • U.S. Postal Inspection Service

2. Identify the specific items of information in your SageStream consumer report that are being reported due to the identity theft. If you would like to request a copy of your SageStream consumer report, please click Consumer Report for instructions.

3. A statement that the Fraudulent Information does not relate to any transaction in which you are/were involved.

For security purposes, we must confirm your identity.  Please see below for additional steps.
4. With your written submission, please also include the following:

A. Required Information

  • Your first name, middle name, last name, suffix (if applicable)
  • Your current address including ZIP code
  • Your current phone number with area code
  • Your Social Security number
  • Your date of birth (month, day, and year)

B. Proof of Identity Documentation

For security purposes, to confirm your identity, please include with your written request:

  • a legible copy of your current driver’s license or other current government-issued identification card showing your current address and date of birth.

NOTE:  You will be required to submit additional identity verification documentation under the following circumstances:

  1. If your driver’s license or other government-issued ID is not current or does not show your current address and date of birth:
  • Submit a legible copy of a document from Category 2 listed below along with a legible copy of your driver’s license or other government-issued ID.
  1. If you do not have a driver’s license or other government-issued ID
  • Submit a legible copy of a document from Category 1 and Category 2 listed below.

Category 1

  • Social Security card
  • U.S. passport picture page
  • Alien registration card
  • Military identification card (front and back)

Category 2

  • Recent cable, utility or telephone statement with your name and current mailing address
  • Bank statement with current address
  • Rental lease agreement showing current address
  • Voided personal check with your preprinted current address
  • Notarized letter from your landlord confirming your current name and residential address
  • Mortgage statement with the current address

Please note that the address on the identification card or other identity verification document must match the address provided in your request.

We will process your request, free of charge, within four (4) business days after we receive a complete request with all required information. We will notify you if, upon review, we decline to block the information.
